Dekalb County Veteran Name Application Form

Fill out and return the application along with $75 and the Veteran's DD214 or similar discharge document or other examples provided on the form. The Veteran could have been deployed from Dekalb County or chose to live in Dekalb County at one time. We do want proof that the Veteran was from the area at some point. We are honoring all 6 branches of the military and from all wars. Please let us know if you have any questions.
